Onboarding Forms I-9, W-4, and W-9 Forms are Defaulting to an Incorrect System Form (California)Resolved Jun 17, 2026 · 25 minResolved

This incident has been resolved. The system-level values for the I-9, W-4, and W-9 forms have been corrected. We have confirmed through a review that no employees filled out or were assigned the California form as a result of this issue, so there was no impact to submitted forms or data in your CareerPlug account. We are implementing additional safeguards to prevent recurrence. Thank you for your patience as we worked as quickly as possible to get Onboarding Forms restored! --- [2026-06-17T10:02:12.862-05:00] (identified) We have identified the issue. Our team is manually correcting the form settings so that I-9, W-4, and W-9 forms point to the correct versions for each user. Once the immediate issue is resolved, we will investigate why the form dropdowns were populated incorrectly to prevent recurrence. We will continue to provide updates as we resolve the issue. --- [2026-06-17T09:57:04.309-05:00] (investigating) We've identified that system-level I-9, W-4, and W-9 forms are currently set to an incorrect system form ("California") rather than reflecting each user's actual state or E-Verify status. We are currently investigating why this form is being incorrectly applied across the board. This issue is affecting all users who are using Onboarding Forms (except Classic Onboard). We are treating this as critical and will post updates as we learn more.
